Due to continued growth, this organisation is seeking an empathetic and motivated Support Coordinator to join their close-knit and supportive team.
Your new role
In this permanent, full-time Monday–Friday position, you will support participants to understand, navigate, and effectively implement their NDIS plans. You will build and maintain strong relationships with service providers and community supports, advocate for participant needs, and promote independence and capacity building.
Your responsibilities will include:
Supporting participants to manage their individual needs and develop essential life skills.
Coordinating referrals and resolving service delivery issues.
Liaising with stakeholders to ensure quality outcomes.
Assisting with plan reviews and ongoing goal setting.
What you'll need to succeed
This role is ideal for a passionate and dedicated professional who thrives in a collaborative environment and values being part of a small, dynamic, and family-oriented team. to be successful in this role, you will ideally have:
A Degree or Diploma in a relevant community-focused field
A valid Blue and Yellow Card
Proven experience working in Community Services
Current First Aid & CPR certification
A current National Police Check
A minimum of 2 years’ experience as a Support Coordinator (highly regarded)
Sound knowledge of NDIS processes, legislation, and best practice
