Our client is seeking a Disability Support Facilitator to join their team. They’re a trusted NDIS provider based on the Gold Coast. Your new role
As a Disability Support Facilitator, your key responsibilities will include:
• Deliver comprehensive case management support to NDIS participants.
• Facilitate referrals and coordinate care within a multidisciplinary team.
• Manage a caseload through in-home service delivery.
• Ensure service quality exceeds accreditation standards.
• Accurate, timely documentation and progress reporting.
• Provide effective leadership and supervision to team members
• Oversee staff rostering and allocation in line with the Collective Agreement.
• Liaise regularly with participants and their primary carers to ensure continuity of care
What you'll need to succeed
• You will be both professional and compassionate
• At least 2–3 years in a similar role with experience in working with participants with complex medical needs
• Obtained NDIS screening card
• Great attention to detail, exceptional phone manner and email correspondents
• Work collaboratively in a team
• Proficient in Microsoft Suite and bonus if you have experience with Shiftcare
