Hays Business Support are currently seeking an APS6 Clinical Care Coordinator for a Federal Government agency that delivers specialised mental health and wellbeing services to veterans and their families. This is a meaningful opportunity to contribute to improving the lives of those who have served, while working within a supportive and collaborative clinical environment. The role will be full-time, office-based with flexible service delivery options, and offered as a 24-month temporary contract.
Your new role
Based in Hobart, you will work within a multidisciplinary clinical team delivering high-quality, evidence-informed mental health support services to veterans and their families. As the Clinical Care Coordinator, you will provide short-term, goal-oriented clinical case management to clients with complex psychosocial needs, supporting engagement with appropriate mental health services.
Your responsibilities will include delivering brief interventions, conducting risk assessments, coordinating care and referrals, and maintaining accurate client records. You will build strong relationships with internal teams and external stakeholders, including community services, health providers and government agencies to support holistic client outcomes.
This role will also contribute to intake assessments, caseload management, and program delivery, including psychoeducation and community engagement initiatives. What you'll need to succeed
To be successful in this role, you will have demonstrated experience in clinical case management, mental health service delivery, or a similar environment. In addition, you will be able to:
• Deliver evidence-based clinical interventions and care coordination for clients with complex needs • Conduct risk assessments and develop care plans aligned with best practice frameworks • Manage your own caseload and work autonomously within a structured clinical model • Build and maintain effective relationships with internal and external stakeholders • Demonstrate strong written communication skills, including accurate case noting and reporting • Contribute to a collaborative and multidisciplinary team environment • Participate in continuous professional development and clinical supervision
You will also hold one of the following: • Registration as a Psychologist, Occupational Therapist, or Registered Nurse with AHPRA, or • Accreditation as a Social Worker with AASW, or • A relevant Master’s level counselling qualification with PACFA or ACA registration
