Managing a diverse caseload of complex clients, providing short-term, goal-oriented clinical case management focused on improving engagement in care
Conducting comprehensive assessments, including risk assessments, to identify unmet health, social and wellbeing needs
Developing and implementing tailored care plans that address barriers to treatment, enhance resilience, and support recovery-oriented outcomes
Delivering brief clinical interventions and psychosocial support, such as psychoeducation, motivational interviewing and problem-solving strategies
Building strong, trusted relationships with clients to foster engagement and sustained participation in treatment
Beyond direct client work, you will play a crucial coordination and system navigation role:
Acting as a central liaison between clients and service providers, ensuring seamless referral pathways across government, community and specialist services
Partnering with internal clinicians and external stakeholders to deliver a joined-up, multidisciplinary approach to care
Supporting clients and families to navigate complex systems, reducing fragmentation and improving continuity of care
You will also contribute to the broader service delivery model:
Maintaining accurate and compliant client records, care plans and progress notes using clinical systems
Participating in case reviews, team meetings and clinical supervision to uphold best practice and continuous learning
Supporting program development, quality improvement initiatives and service evaluation activities
